Fall 2008 Recreational Registration Form.  Please complete this form and mail it to the office to register your child for recreational soccer.  Age requirements and fees are listed on the 2nd page of the form.

Medical Waiver Form- All players and parents must sign this form and have it notarized.  PLEASE GIVE THIS FORM TO YOUR COACH AT THE FIRST PRACTICE.  IT IS NOT NECESSARY FOR YOU TO SEND IT TO THE OFFICE WITH YOUR CHILD'S REGISTRATION FORM.  Coaches must have these forms in their possession during all practices and games.   

Recreational Coaches Registration Form - Required by all coaches, coaching recreational teams.  (Updated 9/07)

Application to Play Up- Required for any player who wishes to play on a team older than his or her age group in the Recreational Division. (Restrictions Apply)

Competitive Roster Form

Competitive Player Registration Form- Must be filled out by all premier players wishing to be rostered through SSA.

Premier Coaches Registration Form and Code of Ethics- Required by all coaches, coaching premier teams.

Premier Team Tryout Request Form (Updated 3/28/07)- Required by any team holding open tryouts between June 15 & July 15.

Supplemental Roster - Form needed to make change to your current roster.

Multiple Roster Request Form- Form needed if you wish to be rostered on more then one team.

Guest Player Form- Form needed to borrow a player from another team.  This form is now available to complete online at the KSYSA web site by clicking here.

Tournament Travel Request -  This process is done on-line at www.ksysa.org.  Please see directions under "E-Travel".

Request for Release or Transfer-Must be completed by a player who wishes to transfer off of a team or tryout for a different team, outside of the free agent period.  The free agent period is June 15 - July 15.

3V3 Registration Form & Rules (Updated 4/23/08)- Team registrations only.

These Forms Are Required to Referee:

Referee Service Agreement- This form is the service agreement between SSA and Referees.

Referee Availability Form- This form is necessary for referee scheduling for the Fall 2007 season.
 

Application for use of SSA Properties - Form required to schedule field usage for matches.

Application for Voting Membership - A must for seeking election status on the Board of Trustees, or for voting eligibility.

Refund / Reimbursement - Form needed to request a refund of registration fees (prior to start of season) or other such fees.  Also to request reimbursement of money spent on behalf of the association.

Field Dimensions - Layout Instructions used by SSA.

Accident Insurance Claim - This is the insurance form for the supplemental insurance through KSYSA.

Bylaws - Bylaws of Sunflower Soccer Association.
