Forms for Competitive Play

Fees are as follows:

• Supplemental Roster
• Multiple Roster Request Form
• Request for a Release or Transfer
• Medical Waiver
• At-large player (Kansas Youth Soccer registers all at-large players)

If you have a player who has not registered through Sunflower Soccer in the past we will need a copy of that player’s birth certificate.

All adults on the roster need to complete a disclosure statement for a background check EVERY year.  You will need your coach or team manager login for gotsoccer to complete.
